Application Process: Adult Volunteer: 1. Complete the Adult Volunteer Application Packet (pdf) which includes the required background check forms. All volunteers must complete and clear a background check facilitated by our Human Resource department prior to volunteering. Save the application to your computer and e-mail us, or print and send via mail to: Volunteer Services at Swedish Medical Center, 501 East Hampden Ave., Englewood, CO 80113. 2. Once your application and background check has been approved, we will be happy to extend an invitation to attend a mandatory New Volunteer Orientation. The orientation consists of an introduction to our facility, an overview of the Volunteer Program, as well as information pertaining to understanding and adhering to Swedish Medical Center's Core Values and Standards in delivering memorable care and exemplary service to our patients and visitors. All new volunteers are required to have a TB test, which will be administered by our Employee Health Department, as well as provide documentation of flu vaccination during the winter flu season. Flu shots in addition to TB shots are free of charge for new volunteers. Teen Volunteer: 1. Complete the Teen Application Packet (pdf) comprised of the letter of recommendation, junior contract and background check form. Note: Students under the age of 18 must also provide a copy of their Social Security Card for verification. Save the forms to your computer and e-mail us, or print and send via mail to: Volunteer Services at Swedish Medical Center, 501 East Hampden Ave., Englewood, CO 80113. 2.
